A suspected case of monkeypox in India has been confirmed positive, according to a statement from the health ministry. Laboratory tests identified the presence of the mpox virus, specifically the West African Clade 2 strain, in a 26-year-old man from Hisar, Haryana. The man had recently returned to India from abroad and was admitted to Delhi’s LNJP Hospital on Saturday.

Global Mpox Resurgence and New Strain

The resurgence of mpox, combined with the discovery of a new strain known as Clade 1b in the Democratic Republic of Congo (DRC), has caused concern worldwide. The World Health Organization (WHO) declared its highest level of international alert on August 14. The Clade 1b strain was first detected in September 2023 among sex workers in eastern DRC. It has since been identified in neighboring countries like Burundi, Kenya, Rwanda, and Uganda, as well as in Asia and Europe.

However, India has not yet reported any cases of mpox caused by this new strain.

Preparedness in Delhi Hospitals

Three government hospitals in Delhi—Lok Nayak Jai Prakash (LNJP) Hospital, GTB Hospital, and Baba Saheb Ambedkar Hospital—have set up isolation facilities for suspected and confirmed cases of monkeypox. These precautions are part of India’s efforts to control potential outbreaks.

Symptoms and Stages of Monkeypox

Monkeypox symptoms include fever, headaches, muscle aches, and swollen lymph nodes. These are followed by a rash that develops in stages: starting as macules, progressing to papules, vesicles, pustules, and finally scabs. The rash usually begins on the face and spreads to other areas of the body.

Transmission and Prevention

The virus can be transmitted through direct contact with infected animals, humans, or contaminated materials. Respiratory droplets can also spread the virus, especially during prolonged close contact. To prevent the spread of monkeypox, it is important to practice good hygiene, such as frequent handwashing. In healthcare settings, the use of personal protective equipment (PPE) and isolation of infected individuals are key strategies in controlling outbreaks.

The Indian government has issued an advisory on monkeypox following the World Health Organization’s (WHO) declaration of a Public Health Emergency of International Concern (PHEIC). Union Health Secretary Apurva Chandra directed the advisory to States and Union Territories, emphasizing the clinical profile of the infection. According to the advisory, most cases of monkeypox involve young males, with a median age of 34 years, primarily between 18 and 44 years old.

Globally, WHO data shows that 96.4% of monkeypox cases are male, and the median age of infection remains 34 years. Notably, males aged 18-44 account for 79.4% of reported cases outside the African region. A 2023 study also highlighted that 96.4% of patients were male, with 84.2% identifying as bisexual, gay, or men who have sex with men.

Sexual contact remains the leading mode of transmission globally, followed by non-sexual person-to-person contact. Despite this global concern, India has not reported new cases in the current outbreak. All suspected cases tested at the National Institute of Virology (NIV) Pune have come back negative, although the Ministry of Health continues to monitor the situation.

A new strain of the monkeypox virus has been identified this year, spreading beyond its original presence in the Democratic Republic of Congo to other East African countries, including Burundi, Kenya, Rwanda, and Uganda.

Monkeypox symptoms include swollen lymph nodes, fever, headache, body aches, and profound weakness.

Health officials in Vietnam’s Ho Chi Minh City announced the country’s first case of monkeypox on Monday, according to the state-run Vietnamnet newspaper, without providing any details on the patient.

According to the reports, Mainland China reported its first case of monkeypox last month in the western megacity of Chongqing in a traveller who arrived from abroad.

The traveller arrived in China on an unspecified date, according to the Wall Street Journal, citing the Chongqing health commission. The traveller developed a rash and other symptoms while being quarantined in accordance with COVID-19 controls, a practice that affects anyone arriving from abroad, according to the WSJ.

After reviewing test results, the Chinese Center for Disease Control and Prevention confirmed the monkeypox case. According to the report, the traveller is being treated at a designated hospital and is in good condition.

According to state media reports, China has detected its first monkeypox infection in a patient quarantined for Covid-19 in the southwestern city of Chongqing, but the risk of community transmission is low.

“The patient was an international arrival, and the monkeypox was discovered during Covid-19 quarantine,” according to the reports by health officials.

The patient’s details, including gender and age, have not been released, with authorities only stating that the individual had flown in from abroad.

As per the reports, “the patient is being treated in isolation at a designated hospital and is in a stable condition.”

“Experts concluded that the patient had been isolated in quarantine upon entry into Chongqing and had no record of social activities. The risk of … an epidemic is low,” the statement, as per SCMP, said.

The World Health Organization declared the monkeypox outbreak a global public health emergency on July 23 after cases were discovered in countries where the infection is not endemic, such as Europe and the United States.

According to a recent report, Hong Kong, which is governed by Beijing as a special administrative region, reported its first monkeypox case on September 6 in an international traveller flying from the Philippines.

Since the disease’s outbreak in other countries, Chinese officials have developed diagnostic and treatment guidelines, as well as prevention and control guidelines.

Chinese experts had previously told that it was only a matter of time before the mainland detected monkeypox cases, but that “…given the disease’s limited routes and speed of transmission, the disease is unlikely to cause major outbreaks.”

The Union health ministry reported on Tuesday that a 31-year-old Nigerian man who had been admitted to Delhi’s Lok Nayak Hospital had tested positive for monkeypox. It was also noted that the patient had no prior history of international travel. While India has already recorded eight cases of monkeypox, this is the third to occur in Delhi.

Union health minister Mansukh Mandaviya said on Tuesday, “India currently has eight confirmed cases of monkeypox, out of which five patients have a history of foreign travel.”

Five of the eight cases of monkeypox, according to the health ministry, have been reported in Kerala. A 22-year-old man died on Saturday after returning from the United Arab Emirates, and Kerala on Monday confirmed India’s first monkeypox fatality when his test results came back positive. The state’s authorities were looking into whether monkeypox was the only factor in his demise.

A task force was established by the centre on Monday to keep an eye on the disease, which can be spread through direct contact with bodily fluids or the lesions of an affected person. Indirect contact with lesion-related materials, such as tainted clothing or linen, can also spread the disease. 75 nations have so far recorded at least 22,000 instances of the virus.

According to Kerala’s state health minister, Veena George, one new case of monkeypox has been confirmed in the state. A resident of Malappuram was reached on July 6 and was receiving care at the Manjri Medical College.

The monkeypox virus has been detected in a 35-year-old man who travelled to Kerala from the UAE earlier this month. He is the third person to have contracted the virus from either the nation or the state.

The minister added that everyone who has touched the patient directly is being strictly watched.

Monkeypox has been rapidly spreading throughout the world since early May.

According to the World Health Organisation, clinically less severe than smallpox, Monkeypox is a viral zoonosis (a virus that spreads from animals to people). It has symptoms that are comparable to those of smallpox. Monkeypox has replaced smallpox as the most significant orthopoxvirus for public health since smallpox was eradicated in 1980 and smallpox vaccinations were subsequently discontinued. Primarily affecting central and west Africa, Monkeypox has been spreading into cities and is frequently seen close to tropical rainforests. Numerous rodent species and non-human primates serve as hosts for animals.
